Attention deficit disorder (ADD) may be a possibility but it might simply be a matter of your needing to learn more relevant organizational and time management skills. You may need to broaden your knowledge in these areas. Counseling could provide that type of knowledge and guidance.
You described being nervous and unfocused during presentations. Many people can relate to that experience. Public speaking is a very common fear. Some people are more frightened of public speaking than they are of death. Anxiety and nervousness could easily make you forgetful during presentations.
